Monthly Behaviour Tracker Templates for Classroom Data Collection

Support your behaviour‑tracking routines with this pair of Printable Monthly Behaviour Tracker Templates, designed to help teachers monitor a single student’s progress toward a specific behaviour goal over the course of a month. These trackers make it easy to collect consistent data, spot patterns, and communicate progress with families, support teams, or administration.

Each template provides space for daily scoring and specific comments, giving you a clear snapshot of how the student is progressing toward their goal.

A Simple, Effective Way to Track One Behaviour Goal

These behaviour trackers are designed for quick, daily use. Each page includes:

  • a behaviour goal section
  • a daily scoring grid
  • a comment box for each day
  • a key to guide consistent scoring

To use the tracker:

  1. Use the key at the bottom of the page to guide scoring.
  2. Circle a number each day based on the student’s behaviour:
    • 3 – Always
    • 2 – Sometimes
    • 1 – Rarely
  3. Add comments when needed to note triggers, successes, strategies used, or patterns you observe.

These trackers are ideal for behaviour plans, intervention documentation, parent communication, or progress monitoring.

Download Your Monthly Behaviour Chart Pack

Your printable behaviour chart pack includes two versions of the behaviour tracker:

  • Version 1: A tracker with the months already listed—simply circle the month you’re using.
  • Version 2: A tracker with a blank space to write in the month, offering more flexibility.

Both versions are clean, simple, and easy to print for repeated use throughout the school year. They are available in both PDF and editable Google Slides formats. To get your copy, click the dropdown arrow on the download button and select your preferred file type.
